Thank you for trusting GD Dentistry with your patients. We work alongside referring dentists and physicians across New York and Connecticut, handling the surgical and implant portion of care and returning each patient to your practice for ongoing treatment. Our goal is simple: make the referral easy for you, and make the experience excellent for your patient.

How to Refer a Patient

You can send a referral in whichever way fits your workflow best:

• Fill out the online referral form below.
• Call the GD Dentistry office nearest your practice and ask for the referral coordinator.
• Send us the patient’s name and phone number, and our team will contact them directly to schedule.

At minimum we need the patient’s first name, last name, and phone number. If you have imaging, treatment notes, or a specific concern you would like evaluated, please include those as well.

What We Treat

Referring doctors most often send patients to us for:

• Full arch implant treatment, including All-on-X and fixed teeth in 24 hours
• Single and multiple dental implants
• Zygomatic implants for patients with significant bone loss
• Bone grafting and sinus lift procedures
• Complex extractions and surgical cases
• Periodontal treatment and implant restoration
• IV sedation for anxious or medically complex patients

What Your Patient Can Expect

Every referred patient receives a comprehensive consultation and CT scan at no cost. We review the findings with them, explain their options in plain language, and provide a written treatment plan before anything begins.

We keep you informed throughout. You will receive documentation of the treatment performed, and the patient returns to your practice for restorative and routine care unless you have asked us to handle that as well. We do not compete for your patients. We complete the part of their care you have referred to us and send them back.
